How do I add, change or remove beneficiaries from my account?

You can add, change or edit beneficiaries by completing this Customer Information Change form. To add a beneficiary, we require the following information for each beneficiary:

• Name

• Address

• Date of Birth

• Social Security Number

• Phone Number

At least one account owner is required to sign requesting a change in beneficiaries. Beneficiaries do not need to sign or provide a copy of ID.
